The Kitsap Aging & Long-Term Care main office location is within the Givens Community Center, 1026 Sidney Ave in Port Orchard, WA. The Givens Center is closed to the public; however Aging & Long-Term Care phone and email services are available during business hours, Monday-Friday, 8:00am- 4:30pm by calling (360) 337-5700 or (800) 562-6418. There is also an office located at the North Kitsap Fishline building, 19705 Viking Ave NW in Poulsbo, WA 98370. If in-person services are preferred, please call (360) 337-5700 to schedule a walk-in appointment (both office locations). Email inquiries to SeniorInfo@kitsap.gov
